The Records Request Department can assist you in receiving records if you are a provider, a community partner, a legal guardian, or your own records (for clients over 14). Please download the Release of Information (authorization to use and disclose protected health information) form below. The release of information must be completed by the client’s legal guardian or self (if over 14). Please fax completed form along with a request for specific records to 503-233-4359.

You can contact your clinic location directly by visiting our Locations page and requesting records from our administrative team. They will assist you in completing the records request form via phone or email you a copy to complete and return. You may also download the records request form HERE and return the completed form to the administrative team at your clinic.
