Central Intake
The Central Intake Department screens and schedules intake appointments for the five outpatient clinics, as well as providing resource and referral information for callers state-wide.

Referral Forms
DHS Referral Form - this form is used to request outpatient mental health services for children in foster care. It is completed by any DHS branch and faxed to Central Intake for follow up by Morrison with the caregiver.
General Outpatient Referral Form -
Multnomah County Health Department Referral Form - this form is used by any Multnomah County Health Department clinic to refer a medical patient to Morrison for outpatient mental health services. It is faxed to Central Intake for follow up by Morrison with the caregiver.
